For images, click on Training Elements then on Assessments. Find the assessment that you want to modify. Choose the question and click Edit or create a new one. From there, it's just a matter of selecting the image that you want to embed.
For all other file types, the content needs to first be uploaded into your Materials section. Once uploaded, click on the item and navigate to the Courses & Assessments tab and copy the placeholder.
In your assessment question, enter the placeholder into the text area. It can be added into both the question and answer option fields.
